Martin – Mechanic – Davie, Florida

Meniscus Tear, Knee Pain, Knee Replacement

A Florida Mechanic experienced chronic pain after having knee replacement surgery on both knees. His work required moving, stooping, and lifting, and his condition eventually became too severe for him to continue working. He was initially approved for Long Term Disability benefits. But suddenly, after receiving more than five years of monthly benefits, Lincoln Financial sent him a letter indicating he no longer met their definition of disabled and monthly benefits would stop.

Shocked at the sudden loss of income, he contacted Fields Law Firm for help. The attorneys at Fields Law contacted our client’s treating physicians, and ordered an independent vocational report to illustrate the serious nature of our client’s condition. Fields Law Firm submitted a strong appeal to the insurer and kept our client updated every step of the way. Ultimately, Lincoln Financial overturned their original decision. Our client received a lump-sum benefit for back-pay and had his monthly benefits reinstated.

Donald – Mineral Processor – Ocala, Florida

Headaches, Severe Pain, Anxiety, Depression

A mineral processor from Florida began having intense migraines and back pain. He attempted to continue work, but he started having blurred vision and was forced to stop working. He was paid Long Term Disability benefits for eighteen months. Prudential then sent him a letter stating that his benefits would end because he did not meet their definition of disabled.

Worried about his lost income, he contacted Fields Law for help with an appeal. The attorneys at Fields Law built a strong case for our client. They compiled updated reports from our client’s doctors, and reviewed the case file. Ultimately, Prudential reinstated Long Term Disability benefits for our client. With his monthly income restored, our client was relieved to be able to continue treatment without the stress of fighting Prudential.

Siri – Sales Associate – Greenacres, Florida

Degenerative Disc Disorder, Chronic Knee Pain, Anxiety, Depression, Panic Disorder

A Sales Associate from Florida applied for Long Term Disability benefits with an insurance company provided by her employer after suffering from degenerative disc disorder, chronic knee pain, anxiety, depression, and panic disorder. Her Long Term Disability benefits were originally approved by the insurance company. However, Lincoln Financial terminated benefits when she relocated to a more affordable part of the country and had not yet established care with new providers.

The attorneys at Fields Law Firm contacted both the old and new medical providers, obtained assessment forms and letters demonstrating her doctor’s support. Based on those statements of support, updated medical records and the convincing legal analysis in our appeal memo, the attorneys at Fields Law put together an appeal that Lincoln Financial could not dismiss. Our client was receiving benefits again two months after the appeal was filed. She was happy to be able to focus on her medical care and not have to keep struggling with Lincoln Financial.
